We selected the 36" bamboo table for the extension for the exact reason you mention....it leaves an opening at the door, and it was super light and folded up so one person could grab/lift it out of the way quickly & easily..

We've built up our sleeping pads with enough memory & latex foam that they're now high enough to be level with the cool cat.  We got it at Plow-N-Hearth (http://www.plowhearth.com/ggl-search.asp?q=bamboo+bench&submit.x=0&submit.y=0&search_type=normal
We liked it because it could do double duty as an outdoor table.

We have the clamshell as well.  I'm 5'4" and DH is 6'2" - we sleep wheel to wheel (I get the inside).  We were actually quite comfortable even with our 10-month-old daughter between us (but we were sleeping head-to-toe).  If it weren't for the cool cat being so high, an extension that leaves an opening at the door would be nice. 

There is a space in the front bench for a porta-potty (it just requires moving the seat pad out of the way to access it).  We don't use one (just hike out to the outhouse), but we did get a potty/shower tent.
