*PAPER TEMPORARY REGISTRATION NOT ACCEPTABLE.

7. Copy of Declaration page of Commercial truck insurance
showing a minimum of $500,000 liability insurance and Hoosier RV Transport
as additional insured. Binders not accepted.

8. Digital camera capable of emailing photos must be carried at
all times.

9. Cell phone

10. Emergency equipment

11. Information for orientation ( mandatory: complete names,
addresses, phone numbers and dates employed for the last 10 years)

**Failure to bring the above documentation will delay orientation**

Note: If you live in CA, NH, PA or WA, you MUST bring a current Motor
Vehicle Report from your license branch with you obtained within the last
30 days.

**Note** You need to have a CDL-A license if the combined GVWR sticker
rating on your truck and trailer you are transporting is over 26,000#.

HOOSIER RV TRANSPORT:

pays top rates.

gives 50% advances

balance of pay immeditaly upon turning in required paperwork.

provides cargo insurance and liability coverage while under load. Provides
legalization for all US and Canadian loads.

offers log orientation classes and updating log classes.

has required orientation classes where you will be provided a copy of THE
FEDERAL MOTOR CARRIER SAFETY REGULATIONS book.

provides National Safety Council sanctioned classes. Insurance companies in
most states will give a 10% deduction in premium rates for having
satisfactorily passed this course.

uses the PC*Miler program to compute accurate "practical" mileage for all
runs.

Hoosier recognizes that you are an independent owner operator responsibile
for your trucking business operation. You lease your equipment on with us
and provide your services to deliver RVs. Therefore, we do NOT deduct taxes
but will issue a 1099 at the end of the year.

REQUIREMENTS ARE AS FOLLOWS:

Must have at least 2 years commercial towing experience (not personal).

Must be at least 26 years old.

Have the proper driver's license for your state.

A CDL-A is required if your truck and the trailer that you are transporting
are rated over 26,000#s GVWR. Call us if you have any questions.

Have a passport or application in progress.

Have an excellent driving record.

Never had a DWI, DUI, OWI, etc.

Have a current D.O.T. physical (there is a local clinic here.)

Be able to pass a drug screen.

Current D.O.T. truck inspection. There are several inspection stations near
our terminal.

Valid truck registration/title.

Have the required D.O.T. emergency equipment.

Must be able to pass a road test.

ALL CELL PHONES MUST BE HANDS FREE! Non-compliance will cost fines starting
at $2,700 and $11,000 per occurance!.

Required to attend our National Council Safety Course within 30 days of
leasing on OR have proof of having passed an NCSC certificate issued within
the past three years by another company.

Must have a copy of your $500,000 unladened liability insurance policy
showing Hoosier RV Transport as additional insured.

Owner operators with trucks having a GVWR over 26,001# will be responsible
for their quarterly fuel taxes.

Lessor acknowledges that as provlded in lndlana Code, lC 22-3-6-1(b)(8) an
owner-operator providing a motor vehicle and/or driving services under a
written contract with a carrier that is subject to the leasing regulations
of the Federal Motor Carrier Safety Administration ("FMCSA") is not covered
by the worker's compensation laws of Indiana; (2) states that he or she is
not an employee of Lessee and (3) otherwise elects not to be covered under
the state workers' compensation Iaws.

$100 deposit for your transport plates. We will be glad to deduct $25 from
your first four trips' settlement checks. THIS DEPOSIT IS REFUNDED
IMMEDIATELY WHEN YOU DELEASE AND TURN IN YOUR TWO PLATES.

A damage escrow account is required for every owner operator. This account
is to cover damages you are responsible for. NO MONEY IS NEEDED UP FRONT.
Single trucks escrow accounts are $2,000, multiple trucks (haul & tow
trucks and lowboys) are $3,000. We will deduct 10% out of each disbursement
check until you have satisfied your escrow account.
